Greetings ALA Athletes and Families!!

This is Savannah Martinez, current Head Athletic Trainer at ALA Ironwood, and I am reaching out today to bring you some awesome news!! Through my exclusive partnership with Banner Health and Sports Medicine, we have collaborated to bring healthcare specialists on campus for one day only to help medically clear athletes' for sports by providing pre-participation physicals for the upcoming school year at the low price of only $15!! For the first time EVER, we will have Sports Medicine Doctors, Orthopedic Surgeons, Physical Therapists, and Athletic Trainers together on campus to assist with the physical process and evaluation. This will also give special, one on one time with healthcare providers to help answer any questions or concerns you may have about your athlete's health and future in sports. I am so grateful to be able to gift my athletic population and their families with this limited-time offer. FYI: Physicals completed before March 1st, 2024 will NOT be valid for the next  upcoming school year. Only Physicals done after March 1st, 2024 will be considered valid for this upcoming school year. 

ALA Ironwood MS/HS Physicals

When: Friday May 3rd, 2024

Time: 2:30-5:30pm 

- We will NOT accept any athletes past 5:30pm!

Event Ending: 7pm 

Where: @ ALA Ironwood

Cost: 15$ CASH ONLY!!

- We will not collect checks or electronic payments. CASH ONLY!!

Consent to Treat:

Please have ALL AIA evaluation paperwork completed prior to arriving with Parent/Guardian signatures on Page 5. Verbal consent will not be accepted. 

- Documents can be found online on Register My Athlete, the AIA, or ALA athletics website.

• We will have some physical copies as well. 

- High School athletes may attend physical without parent/guardian, if ALL paperwork is completed and signed. 

- Paperwork will be checked via Register my athlete or physical documents and NO student will be checked in without proper documents signed. 

Registration

Student Athletes MUST be in line to check in by the end of advertised time, 5:30pm. Doors will be closed and no one else may get in check in line! This is to respect the committed time of all of our healthcare providers and volunteers.

- Event will run on a First come, First serve basis. Those who plan payment, complete necessary paperwork ahead of time, and arrive early will have their efforts respected and reciprocated with timeliness during the event. 

My Healthcare Providers and I are extremely excited to give our full time and attention to you and your athletes while helping to ease the process of physicals before heading into the upcoming sports seasons!!
